Hospices that have fewer than 50 survey-eligible decedents/caregivers in the "reference period" may request an exemption from participation in the CAHPS Hospice Survey.
Hospice agencies must authorize an approved CAHPS Hospice Survey vendor to submit data on their behalf for the administration of the CAHPS Hospice Survey.
All hospices must have user account information and be granted access to the CAHPS Hospice Survey Data Warehouse.
The Centers for Medicare & Medicaid Services (CMS) offers CAHPS Hospice Survey Training in the Fall of each year. The training is intended for hospices, survey vendors that participate in the CAHPS Hospice Survey, subcontractors who are responsible for major functions of CAHPS Hospice Survey administration, as well as other interested individuals and organizations.
The Training Registration Form was available until 5:00 PM EDT on Wednesday, September 4, 2024. No additional registrations will be accepted.
The new Informational Flyer for use by hospices to inform caregivers of the CAHPS Hospice Survey is meant to be included in bereavement packages provided to family members after patient death. The wording on the flyer must be used as provided. The flyer is available here.
