The CAHPS Hospice Survey Instruments have been approved by OMB.
English language materials must be the default language in the continental U.S. and Spanish language materials must be the default language in Puerto Rico.
CMS strongly encourages hospices with a significant caregiver population that speaks Spanish, Chinese, Russian, Portuguese, Vietnamese, Polish, or Korean to offer the CAHPS Hospice Survey in all applicable languages.
The CAHPS Hospice Survey Project Team has posted the Quality Assurance Guidelines V11.0 English, Spanish, Traditional Chinese, Simplified Chinese, Russian, Portuguese, Vietnamese, Polish, and Korean version of the Mail Survey Materials, Prenotification Letter, Web Survey Materials, and Informational Flyer.
The FAQs, Interviewing Guidelines Telephone, Telephone Script, and Examples of Supplemental Questions are available in English and Spanish.
