The CAHPS Hospice Survey Instruments have been approved by OMB and are available in mail and telephone scripts. English language materials must be the default language in the continental U.S. and Spanish language materials must be the default language in Puerto Rico. CMS strongly encourages hospices with a significant caregiver population that speaks Spanish, Chinese, Russian, Portuguese, Vietnamese, Polish, or Korean to offer the CAHPS Hospice Survey in all applicable languages.
CAHPS Hospice Survey mail materials are available in English, Spanish, Traditional Chinese, Simplified Chinese, Russian, Portuguese, Vietnamese, Polish, and Korean. There are three response options for each mail survey (square, oval, and circle). The cover letters for the initial and follow-up mailing along with the OMB Paperwork Reduction Act Language are provided in each translation.
